What are the required compliance items?

Admission Compliance Documents:

  1. An Axon Education application
  2. A government issued photo ID
  3. Proof of graduation from High School or a GED
  4. A passing pre-admission assessment score and written essay response
  5. A video essay
  6. A signed Enrollment Agreement
  7. In some instances, a signed monthly payment plan agreement


Clinical Compliance Documents: 

  1. Personal Fitness Attestation signed by the student
  2. Ten-Panel Drug Screening (with negative results)
  3. Certificate of Completion of an American Heart Association approved Basic Life Saving (BLS) course. (Exception: may be completed at the first face-to-face skills lab) (See CPR Knowledge Base article for list of acceptable CPR certifications)
  4. TDaP vaccination (Tetanus/Diphtheria/Pertussis)
  5. MMR (Measles, Mumps, and Rubella) vaccination
  6. Varicella (Chickenpox) vaccination
  7. Hepatitis B vaccination
  8. TB-Tuberculosis test (skin test or chest x-ray)
  9. Influenza


All immunizations and vaccinations must be current at the date of application and must remain current for the duration of the student’s involvement in the program.
